    360Health Services Term and Conditions

    The Life Assured may avail 360Health services, a health and wellness awareness and education solution designed to help you become more aware of how to better manage your overall health. 360Health services is being offered through third-party service provider(s) associated with PNB MetLife India Insurance Company Limited (PNB MetLife). Third party service provider(s) may at any point of time in future modify, suspend, or discontinue any 360Health offerings.

    The 360Health Services shall be subject to the following terms and conditions:

    • The services shall be offered strictly subject to the availability of suitable third‑party service provider(s) as associated with PNB MetLife. PNB MetLife acts solely as a facilitator of access to the 360Health Services and shall not be liable for any act, omission, negligence, deficiency, delay, or default on the part of any third-party service (s) provider in the delivery or quality of services rendered.
    • Wherever applicable, a primary diagnosis must be obtained from a duly registered Medical Practitioner.
    • The eligibility of the Primary Life Assured and Family Member(s) shall be determined by third party service provider(s). Any changes shall be implemented uniformly and without discrimination across all existing and new customers of the life insurance policy.
    • Premiums payable under the Policy shall not be linked to the offering or availing of any 360Health Service by the Policyholder, Primary Life Assured or Family Member(s) or both.
    • In case the Primary Life Assured and Policyholder are different, services will be only available to the Primary Life Assured unless the Primary Life Assured has expressly opted to extend such services to eligible family member(s), where permitted, in accordance with the Policy terms and applicable conditions.
    • The Primary Life Assured, may at their sole discretion, avail the services for themselves or for eligible Family Member(s) covered under this Policy.
    • The services can be availed only when the Policy is In-Force/Fully Paid-Up Status, as applicable.
    • PNB MetLife & Third party service provider(s) reserves the right to modify, replace, suspend or discontinue any of the 360Health Services or to change the associated service provider(s) at any time.
    • Use of the 360Health Services and wellness benefits shall be undertaken in good faith, with honesty and integrity.
    • Fraudulent Activity
      • The Primary Life Assured and/or Policyholder/enrolled family members shall not encourage, indulge in, assist or act in connivance with any person in any fraudulent activity, whether directly or indirectly, relating to the use of the 360Health Services for personal gain, revenue generation or any commercial or public purpose.
      • Fraudulent activity shall be determined in accordance with the service provider’s internal policies and applicable laws, and may include but not limited to, without limitation, misrepresentation, concealment or suppression of material facts, or furnishing of incorrect, false or misleading information
      • In the event of any fraudulent activity, the service provider(s) shall be entitled to pursue all remedies available under the applicable Laws..
      • Further, the service provider(s) may, at their sole discretion, permanently suspend, restrict or discontinue access to the 360Health Services, including any ongoing or future service requests. In such circumstances, PNB MetLife shall have no liability or responsibility whatsoever in relation to the continuation, suspension or discontinuation of such services.
        Identification/Commission of any fraud or misrepresentation shall result in immediate cessation of access to the 360Health Services and further appropriate action under the applicable laws.
      • PNB MetLife only facilitates access to the third party service provider (s) platform through its website, digital platforms, Khushi App or any platform as and when notified and does not supervise, manage, control, endorse, assure or assume responsibility for any health or wellness services, including but not limited to tele‑consultations, in‑person medical consultations, diagnostics, medicine delivery, laboratory test bookings, vaccination services or other professional inputs provided by third party service provider (s) or its medical, nutrition, fitness or wellness experts. PNB MetLife does not represent, warrant or guarantee the accuracy, effectiveness, suitability or appropriateness of any medical, dietary, fitness, supplement‑related or wellness advice, recommendations or regimes made available through by third party service provider (s), and shall have no role, responsibility or liability in relation thereto. Any concerns, complaints or disputes arising out of or in connection with such services shall be addressed exclusively with by third party service provider (s).
    • Privacy Notice and Consent:
      By using 360Health, you will be supported by third party service provider(s) associated with PNB MetLife, for health & wellness services as provided by third party service provider(s).
      • You acknowledge and provide your free, specific, revocable, informed and unambiguous consent to third party service provider(s) associated with PNB MetLife, to collect and process your personal data, and where applicable, the personal data of your declared dependent such as identification details, contact information and health related information solely for enabling medical consultations and health and wellness services facilitated through third party service provider(s) associated with PNB MetLife. In cases where any family member or dependent enrolled under the policy is a minor, the primary life assured / policyholder, acting as the parent or lawful guardian, hereby provides free, specific, informed and unambiguous, revocable consent on behalf of such minor for the collection, sharing and processing of the minor’s personal data, including health data, and for availing the 360health services. such consent shall be issued by the customer in their capacity as guardian and shall apply solely for the limited purpose of enabling access to the services through the relevant third party service provider(s), in accordance with applicable law.
      • PNB MetLife retains personal data only for such duration as is necessary to enable access to the 360Health platform or as required under applicable insurance, legal or regulatory obligations. third party service provider(s) determines and applies its own data retention practices in respect of health and wellness data, as detailed in its Privacy Policy. Your personal data will be stored, processed and shared strictly for this purpose in accordance with the respective Privacy Policies available at https://www.pnbmetlife.com/quicklinks/privacy-policy.html and https://getvisitapp.com/privacy-policy/ which describe in detail the categories of data processed, retention practices, security safeguards, cross border (if any) and your rights under the Digital Personal Data Protection Act, 2023 and other applicable laws and regulatory provisions.
      • You retain the right to access, correct or erase your personal data and to withdraw your consent at any time in accordance with the Digital Personal Data Protection Act, 2023. Such requests may be submitted through the grievance redressal mechanism outlined in PNB MetLife’s Privacy Policy or by contacting the Privacy Office / Privacy Compliance Group at askprivacy@pnbmetlife.com.
      • All such consultations, services and opinions are rendered independently by third party service provider(s) associated with PNB MetLife, in accordance with its terms of service and without any oversight or intervention from PNB MetLife.
      • You further understand and consent that access to these services is determined by the policy you hold, and therefore the services may be available either solely to you or if you are a customer of the Health 360 Plan also to up to 4 dependents, as declared by you at the time of accessing the services. Where dependents are included, you confirm that you are duly authorised to share their personal data and health related information with PNB MetLife and third party service provider(s) for the limited purpose of enabling their access to the 360Health services.
      • PNB MetLife shall not be responsible or liable for any processing of health or wellness data undertaken by third party service provider(s) beyond facilitating platform access, nor for any breach or incident attributable to third party service provider(s) systems, processes, or personnel.
      • Where any additional personal data, including health data, is requested directly by any third party service provider(s) for enabling or continuing the 360health services, the service provider(s) shall be responsible for obtaining separate, explicit consent from the primary life assured / policyholder / eligible family member(s), as applicable. PNB MetLife shall have no role, responsibility or involvement in seeking, managing or validating such additional consents.
      • By procuring and availing the non‑insurance value‑added 360Health Services, you provide your free, specific, revocable and unambiguous consent to the third‑party service provider(s) to share or reverse‑transfer limited personal data, including where applicable health and wellness‑related information of the Primary Life Assured and/or eligible Family Member(s), with PNB MetLife strictly on a need‑to‑know and purpose‑limited basis, solely for evaluating service suitability, enabling and improving the 360Health Services, service monitoring, audit, customer support, grievance redressal and compliance with applicable laws. Such reverse data sharing shall be undertaken only upon obtaining your explicit consent in accordance with the Digital Personal Data Protection Act, 2023 and applicable cybersecurity guidelines. PNB MetLife shall not use such data for underwriting, claims determination, pricing or any adverse decision‑making under the insurance policy. You may withdraw such consent at any time, subject to applicable legal or regulatory retention requirements, and acknowledge that such withdrawal may impact access to the relevant services.

    For any ongoing, serious or persistent health concerns, the Primary Life Assured and/or Family Member(s) are advised to consult directly with a duly registered Medical Practitioner, as the 360Health Services are intended only to provide limited wellness‑oriented support and facilitation and may not be sufficient or appropriate for addressing complex, chronic or emergency medical or persistent health conditions.

    By availing any 360Health Service, the Primary Life Assured and/or eligible Family Member(s) or both shall be deemed to have read, understood, and accepted the detailed terms and conditions as published on the Khushi app platform.

    If you any concerns, queries or grievances in relation to the collection, use, sharing or processing of personal data in connection with the 360health services, or if the customer wishes to review the applicable privacy notice and/or terms and conditions, or to exercise their right to withdraw consent in accordance with applicable law, the customer may write to or raise their request at indiaservice@pnbmetlife.co.in such requests shall be addressed in accordance with the digital personal data protection act, 2023 and the applicable privacy policies
