- Claimant Statement (downloadable form) or written intimation from claimant mentioning policy details and date of death.
- Death certificate issued by Municipal Corporation / local authorities (In absence of the same following documents can be submitted viz: Death certificate / Report issued by Police Authorities/ Death certificate / report issued by Armed forces.
- Nominee ID, residence proof and account details / bank certificate of the nominee.
